Festival Guest application form

Hello! If you'd like to be a guest at our May festival in Portland, please fill out this form. The information gathered here helps us plan our event. We are looking for Filmmakers, Actors, Authors, Artists, Scholars, etc. No one sees this information unless explicitly stated in the instructions.

Your name, or if you are filling this form out on behalf of someone else, their name. If welcomed as a guest, this name will be shown in the guest listing on the website and in the program.

Please enter your best direct contact e-mail address. This will not be listed or disclosed to the public, it is only for communication between you and us (the festival organizers). This will be our primary form of communication with you, so please type carefully.

The best phone number to reach you at, day or night. We will only bug you by phone if we have to. Most communication will be by e-mail, but please include a phone number, just in case (e-mails sometimes get lost, or bounce back, and if we don't have an alternate means of communication, we won't be able to get in touch).

This can be a link to your own Web site, or if you don't have one, it can be a link to your Amazon book listing, IMDB entry, Portfolio, etc. Anyplace on the web where someone (including us) can find out more about you is a safe bet. It's not required, but do try to fill in this field, if at all possible. (limit this to one website please.)

Please enter a plain text, single paragraph biography in third person. If you are welcomed as a guest, this will be used both on the festival web site and in the festival program/newspaper. Ideally, your bio would be about 120 words. Longer bios will have to be trimmed, and bios that are too short don't look good in the program. Do not include links or HTML code. If you don't have a polished bio ready to go, enter what you have, you can revise it later. **If you provided a bio for us for the 2012 Festival and would like to use the same one, let us know.

Use this space to concisely list your publishing or film credits. If there is not enough room, list your most notable or most recent credits.

List any recently released works (within the past 12 months) or upcoming releases (within the next 12 months), one per line. These can be books, films, etc.

If you have books, films, or music that you have created that you can bring to the festival to sell, we will do our best to provide a place where they can be sold. If you check this box, we will contact you with further information.

Please mark any days that you can be available to take part in programming, such as panels, readings, Q&A sessions, etc.

Please indicate if we may place you on one or more panel discussions, related to your area of expertise, during the festival. If you are welcomed as a guest to our festival, this will only help us plan panel participants, it will not influence whether or not we accept you as a guest. Please only mark "Yes" if you are comfortable speaking in front of an audience.

If you are an author, would you like to do a book reading? If you are a filmmaker (director, producer, actor, technician), would you like to be involved in a Q&A session related to your work? Please only mark "Yes", if you are comfortable speaking in front of an (friendly and interested) audience.

If you are asked to take part in a panel discussion, reading, or Q&A, do you have already have experience speaking to an audience?
